Transman Bottom Surgery

While the needs of transmen can vary, a common theme is the deep need to make their outer appearance reflect their inner gender identity. For some, this includes bottom surgery, also known as genital confirmation surgery. This is a highly-specialized surgery performed by only a handful of skilled and experienced surgeons in the world.

The first phalloplasty done for the purposes of sexual reassignment was performed on trans man Michael Dillon in 1946 by Dr. Harold Gillies, which is documented in Pagan Kennedy’s book The First Man-Made Man. Since then, the surgery has been refined and improved. The early dilemma was having a functional penis that retained sensation. Transmen seeking an improvement over the existing phalloplasty procedure approached Dr. Peter Raphael with the International Center for Transgender Care. Dr. Raphael worked with Dr. Alan Munoz, a prominent gynecologic oncologist, to develop an innovative procedure called The Centurion.

The Centurion was first performed by Dr. Raphael in 2002. It is normally performed in parallel with a hysterectomy and vaginoplasty. The procedure takes approximately 2.5 hours. A new, innovative and safer vaginoplasty is performed by Dr. Raphael which effectively closes the vagina in a safer and more efficient manner. The clitoris is released at the chordee. The erectile nature of the clitoris is the basis of the new neo-phallus erection post-operatively. Using a complex procedure of freeing ligaments to provide additional support for the neo-phallus shaft, stabilization is achieved as well as prevention of retraction. The labia majora becomes the scrotal sac which are placed testicular implants. Neo-uretha is created using available tissue and begins at the native uretha opening.
